Handbooks and Forms

NOTE: Many of the forms listed below are in MS Word format. Depending on the browser you use, a User Name/Password dialog box may pop up when you click a link. If this happens, just click Cancel in the dialog box, and the form will load.

The instructions below tell WorkGroup Leaders and and Volunteers what to do with these forms. To download a form, right-click it, and then choose to save to a file on your local hard drive. Downloaded forms may then be opened in MS Word, filled in, and sent it electronically as a file attachment to an email note.

Ø Workgroup Safety Guidelines

For use by Leaders and Volunteers. Contains valuable information for planning a work trip, including guidance on work practices, clothing, safety gear, and first aid kits. May be printed for future reference.

Ø Workgroup Registration: Work Group Leader submits online version of the WorkGroup Registration Form, which now includes the Work Group Checklist.

Ø Parental Permission for Sponsor of a Minor

Ÿ Sponsor (adult volunteer) completes online registration for minor.
Ÿ Sponsor and both parents complete and sign form, and give it to the Leader.
Ÿ Leader checks form, verifies signatures, keeps original for use in Mexico, and sends a copy to CUPS Houston office.

Ø Volunteer Release and Insurance Statement
Volunteer first completes online registration. He can then fill in his Volunteer Release and Insurance Statement online, print it, sign it, get witness signatures, and submit to workgroup leader.
